HMO

The HMO of IL plan provides you with medical care from a network of physician and hospital providers. You must select a primary care physician who provides all of your medical care and authorizes any referrals to specialists within the network.

For a list of physicians and hospitals, refer to the provider directory, visit BlueCross BlueShield website at www.bcbsil.com or call Customer service at 1-800-892-2803.

You should also verify with the physician if they participate in the plan and are accepting new patients. Each member of your family may choose a different physician. You may change your physician at any time by calling Customer Service.

It is important to note that if you receive care from any other source without prior authorization from the HMO, your services will not be covered (except in specific emergency situations).

If you have a medical emergency, go to the nearest hospital, even if the hospital is not in the network. After receiving emergency care, you must call your Primary Care Physician and BlueCross BlueShield within 48 hours.
