Dependent Coverage

Eligible Dependents

Individuals eligible for coverage under the City Colleges health insurance plans include:

  • Legal spouse, Civil Union Spouse, or Domestic partner             
  • Eligible child(ren) until age 26 (or until age 30 for military dependents). 
  • Physically or mentally handicapped children (regardless of age), who have been added prior to age 26

The term children includes:             

  • Natural children             
  • Step children             
  • Legally adopted children             
  • Children under employee’s legal guardianship 

​To add an eligible Dependent during Open Enrollment or because of a qualifying life event, complete the Dependent Eligibility A​ffidavit​​​ form at right and return it to the Benefits Division.

Ineligible Dependents

Individuals not eligible for coverage under the City Colleges health insurance plans include:

  • Employee’s parents
  • Divorced spouse
  • Common law spouse
  • Children who are not under employee’s legal guardianship

Certified documents may be requested at a later date to validate dependency status.  These documents can include:

  • Marriage Certificate
  • Civil Union Certificate
  • Birth Certificate
  • Court Order
