Reasonable Accommodation Request Procedure

This information is for employees and applicants for employment. Students should go to the Disability Access Center at their College.
City Colleges of Chicago is committed to fully complying with the Americans with Disabilities Act (ADA) and the Illinois Pregnancy Accommodation Act. In accordance with the ADA and the Illinois Pregnancy Accommodation Act, City Colleges of Chicago provides reasonable accommodation to qualified individuals with disabilities, or conditions or medical conditions related to pregnancy or childbirth in order to perform the essential functions of their​ job, or to participate in the employment application process.        
 
​​Requesting Reasonable Accommodation          
 
The employee or applicant is responsible for requesting a workplace accommodation for their disability or conditions or medical conditions related to pregnancy or childbirth. Employees or applicants should make any and all requests for reasonable accommodations to the Equal Opportunity (“EEO”) Office within a timely manner of the requested accommodation(s). Employees must complete the Reasonable Accommodation Request Form and the Authorization for Release of Information Form, and obtain a letter from a health care provider that provides a clinical diagnosis of the disability. Both forms and the health care provider documentation should be returned to the EEO Office. ​        
 
The Reasonable Accommodation Process          
 
 
Once the employee’s documentation is received, the EEO Office will review the documentation to determine eligibility and evaluate the request. In doing so, the EEO Office may meet with the employee and the supervisor or department head as needed to assess whether any reasonable accommodation would allow the employee to fulfill the essential job functions, unless doing so would impose an undue hardship. Reasonable accommodations will be made in accordance with the nature of the disability and the nature of the employee’s essential work duties. Accommodations are made on a case-by-case basis, taking into account the type of disability and the specific job requirements involved. An accommodation need not be the most expensive accommodation or the specific accommodation requested by the employee as long as it is effective.         
 
The EEO Office will respond to the accommodation request in a timely manner and will inform the employee of the final action taken on the accommodation request, including what, if any, accommodation will be provided. The EEO Office will record the final action taken by City Colleges of Chicago.​         
 
Confidentiality         
 
All information regarding an employee’s or applicant’s disability and the reasonable accommodation request is confidential, with the exception of the employee’s supervisor(s) and other personnel who need to be informed in order to assess requests for, implement, or monitor accommodations. Any documentation will be kept confidential and separate from the employee’s personnel file.
