If you have passed the approved Food Service Sanitation Certification course, you can apply in person for the City of Chicago Food Service Sanitation Manager Certificate at:

Upon arriving at MXC check in with security and ask for directions to the Food Service Sanitation Program.

       
Applications are accepted during the following times:
9 a.m. to 4:30 p.m. Monday –Thursday
9 a.m. to 12:30 p.m. Friday
Applications are not accepted when the Business Office is closed.

Required Documents     

The following PRINTED documents are required in order to complete the application process:

  • A copy of your National Food Protection Manager Certificate.
  • Training Hours Verification Form completed and signed by Instructor/Proctor.   (2018 Rules -Instructor led course must be conducted by approved City of Chicago Instructor.  On-line training course require training hours’ form completed by proctor with proof of completion of an approved on-line training course.  If the training and exam were taken on or before December 31, 2017 then the hours of training, need to be verified by a City of Chicago or Illinois Department of Public Health Instructor.)
  • Valid Picture ID
  • Fee is $52.00 payable by cash or money order only.
  • If a proxy is applying on your behalf, you will need to provide a written note giving your permission to the proxy.  The note must be signed and dated. 

Incomplete applications are not accepted.

Your City of Chicago Food Service Sanitation Certificate will be mailed to your home address approximately 4 to 6 weeks after your application is approved. In most cases your receipt from the Food Service Sanitation Program  will suffice as temporary proof of your Food Service Sanitation Manager Certificate while waiting for certificate to be mailed.

By public transit: Several transit routes run adjacent to the college including: the CTA Blue Line (Illinois Medical District stop) and the CTA bus #157, 126, and 50.
