Talents of Teaching and Indicators – This document explains the Talents of Teaching. It examines the history of the Talents as well as the FC4-approved indicators associated with each Talent.
Classroom Observation Forms
Classroom Observation Guidelines – This document provides an overview for conducting classroom observations by detailing activities that should happen before, during, and after the observation.
Action Plan (Classroom Observation) - This form is designed to provide a formal plan of action for the tenure track faculty member who has received a rating of “Significant Concern” on any portion of their Classroom Observation Form done by the Department Chair (or designee) or member of the Administration. It should be done immediately after the Classroom Observation concludes (since a “Significant Concern” warrants immediate intervention). It should be given to the Tenure Track Faculty Member and discussed in person to go over the terms and details AND given to the TAP Leader in order to be knowledgeable of additional deadlines and requirements.
Action Plan (Second Semester Review) – This form is designed to provide a formal plan of action for the tenure track faculty member whose performance in the Second Semester Review raised concerns regarding their understanding of the Talents of Teaching and/or the development of their Individualized Learning and Service Plan.
Action Plan (Semester 1 or 3 Portfolio Rubric) – This form is designed to provide a formal plan of action for the tenure track faculty member who has received a rating of “Recommended with Action Plan” on a Semester 1 or 3 Rubric.This Action Plan Form should be completed in consultation with the Department Chair and TAP Leader, given to the Tenure Track Faculty Member and discussed in person to go over the terms and details, given to the TAP Leader in order to be knowledgeable of additional deadlines and requirements, and included in the Semester portfolio.
Student Evaluation Forms
Student Single-session Evaluations – Completed by students for classes that only meet for one session (library orientation sessions, etc.)
Nursing Clinical Evaluation – Completed by students in tenure-track faculty clinical(s) (Nursing Only)
